Who this is for. The person at a small independent clinic in England who chooses, or checks, the laboratory the clinic's blood samples go to: the practice manager, the registered manager, or whoever holds the laboratory contract. It is also for the person who is asked, "is our lab accredited?", and wants to answer with a number and a date.

What you will be able to do. Look up every current UKAS ISO 15189 schedule for a medical laboratory whose main address is in England, with its accreditation number, its address as UKAS lists it, the issue date of the schedule and a direct link to it. Groups headquartered outside England that have sites in England are not in the list; their schedules list those sites. You will also be able to read a schedule of accreditation and see which site and which examinations it covers, and check a laboratory yourself, including the sanctions page most people never open.

What is in it that a general answer is not. The full list in its stated scope, read on one named day: every schedule UKAS lists under "Medical Laboratories" whose main address is in England, each one opened and its standard and issue date read from the schedule itself. The organisations that hold several accreditation numbers, one per laboratory or service, and why that matters when you check. The two laboratories that hold a current schedule and also appear on UKAS's sanctions page. The ISO 15189 suspensions and resignations UKAS lists for England, dated.

1. What ISO 15189 accreditation is, as UKAS states it

UKAS is the United Kingdom Accreditation Service, the UK's national accreditation body. Its page on medical laboratory accreditation says: "ISO 15189 accreditation underpins confidence in the quality of medical laboratories through a process that verifies their integrity, impartiality and competence. Assessments under UKAS accreditation ensure labs meet the relevant requirements including the operation of a quality management system and the ability to demonstrate that specific activities are performed within the criteria set out in the relevant standard."

The same page lists the disciplines ISO 15189 accreditation covers: clinical biochemistry, toxicology and endocrinology; haematology and blood transfusion; microbiology, virology, parasitology, serology and mycology; histopathology, cytology and mortuaries; immunology; genetics; andrology; histocompatibility and immunogenetics; and point of care testing.

UKAS defines accreditation in its FAQs as "the formal recognition by an 'accreditation authority' to the technical and organisational competence of a conformity assessment body, to carry out a 'specific service' in accordance to the standards and technical regulations, as described in their 'scope of accreditation'. It provides a means to identify a proven, competent evaluator so that the selection of a laboratory, inspection or certification body is an informed choice."

Three points from the UKAS FAQs shape how to use the list below:

• Accreditation is scoped. "The specific activities and scope they are accredited for are listed on the organisation's accreditation schedule." A laboratory is accredited for what its schedule lists, at the locations it lists, not for everything it does.
• It is reassessed. "UKAS carries out assessments on an annual basis with a full reassessment of the organisation every fourth year".
• UKAS does not rank. "As the UK's national accreditation body, UKAS has to be completely impartial and therefore unfortunately we cannot recommend one accredited organisation over another." This list does not rank either. It is in alphabetical order.

Where this sits among the rules for a clinic: no regulation or regulator guidance quoted in ER Blood's companion "The result-administration regulator map" says that a clinic must use an accredited laboratory. CQC's page on diagnostic and screening procedures does say that "if you use a remote contractor for diagnostic analysis, such as a laboratory company, that provider will also need to register in its own right." Accreditation by UKAS and registration with CQC are separate things, made by separate bodies.

2. Checking a laboratory yourself, in five steps

The order matters: the schedule is the document that counts, so it comes before the certificate.

Step 1. Find the organisation in the UKAS directory. Go to UKAS "Find an organisation" and choose Organisation type "Medical Laboratories". Search by organisation name, or by number in the "Customer number" box if you have it. A region filter is there too, but the region tag is not the same as the laboratory's address (section 4).

Step 2. Open the schedule, not just the result card. Each result links to the schedule of accreditation as a PDF, at an address of the form https://www.ukas.com/download-schedule/<number>/Medical. UKAS: "The presence of a schedule of accreditation on our website, confirms that accreditation for that organisation is current and valid. Absence of a schedule on the UKAS website indicates that the accreditation is no longer in force; where accreditation is temporarily suspended this is also noted."

Step 3. Check the site and the examinations. On the schedule, find the location your samples actually go to, under "Laboratory locations", and the examinations you send under "DETAIL OF ACCREDITATION". A schedule for a group can list many sites, each with its own activities (section 3).

Step 4. Open the sanctions page. UKAS publishes "Organisations under sanction": withdrawals in the past 12 months, imposed suspensions, voluntary suspensions "for the scope shown", and resignations in the past 12 months. On 20 October 2026 the page said it was "Updated 19 October 2026" and that "this list is updated weekly with the situation current at that time. Please refer to the schedules of accreditation for the definitive accreditation status and scope of an organisation." Section 6 lists its ISO 15189 entries for England.

Step 5. If you are shown a certificate, validate it. UKAS: "All UKAS Certificates of Accreditation are issued electronically", validated "through QR code technology and Blockchain". "Any certificate of accreditation bearing the UKAS logo that cannot be verified via our database (accessed through the QR code present on the certificate) should be considered obsolete or questionable." The certificate is "a supporting document to an accredited organisations Schedule of Accreditation, and these documents should be used in conjunction with one another." "UKAS E-certificates do not contain an expiry date."

Then write down what you checked: the number, the schedule issue number and date, and the date you looked. UKAS recommends "that you routinely and regularly check that the accreditation of organisations you rely upon is current and valid", because the schedule "from time to time may be revised and reissued".

3. What a schedule of accreditation shows

Every schedule read for this edition opens with the same fields. Taking one real schedule as the example, Clinilabs Limited, UKAS number 27641, issue 003 dated 27 May 2026:

A schedule for a group looks different in one important way. Spire Healthcare Ltd, number 8314, lists its locations one by one, each with its activities and a code: for example "Spire Southampton ... Haematology, Biochemistry, Blood Transfusion, Microbiology", location B1, and "Spire Portsmouth ... Haematology", location B2. The accreditation for one site is not the accreditation for another site in the same group.

Schedules in England ran from 1 page to 123 pages on 20 October 2026. The longest was number 8285, Barts Health NHS Trust as the host of NHS East and South East London Pathology Partnership. Length reflects the number of sites and examinations listed, and says nothing about quality.

4. Edge cases that catch people out

One organisation, many numbers. The 403 schedules with an England main address belong to fewer organisations, because many hold one number per laboratory or department. The list shows, for example, multiple numbers for "Synnovis Analytics LLP", "HSL (Analytics) LLP", "The Doctors Laboratory Limited" and "University College London Hospitals NHS Foundation Trust". When you check, check the number that covers the site and service you use, not just the organisation's name.

The directory's region tag is not the address. In the directory a schedule can carry several region tags. Spire Healthcare Ltd, 8314, was tagged with nine regions, including Scotland, while its schedule address is in Elstree. Randox Clinical Laboratory Services Limited, 9329, was tagged London and North West as well as Northern Ireland, while its schedule address is in Antrim, so it is not in the England list below. A current schedule and a sanctions entry at the same time. On 20 October 2026 two England numbers had a current schedule in the directory and also appeared on the sanctions page:

• Dartford and Gravesham NHS Trust, 8817: schedule dated 16 July 2026; listed under imposed suspensions with the date 13 April 2026.
• KIMS Hospital Limited, 21162: schedule dated 25 August 2026; listed under voluntary suspensions with the date 22 October 2025.

UKAS says voluntary suspensions are "for the scope shown" and that the schedule is "the definitive accreditation status and scope". Read both, and ask the laboratory which examinations the entry covers. The list below marks both rows.

Moving between standards. One entry in the sanctions page's resignations table, for the University of Oxford Therapy Acceleration Laboratory, records the resignation of its ISO/IEC 17025 (Testing) accreditation dated 14 September 2026 with the note "Testing now performed under ISO 15189 accreditation". Its ISO 15189 schedule, 28562, issued 8 September 2026, is in the list.

Resignations are not failures, and not accreditations either. The resignations table lists organisations "that have voluntarily resigned their accreditation within the past 12 months". A resigned number has no current schedule. If your laboratory's number is there, ask which number now covers your samples.

Sample collection sites. Some schedules name a location whose accredited activity is sample collection, such as "Venepuncture" at Clinilabs' Foley Street address. Of the 403 England schedules, 24 contained the word "venepuncture" or "phlebotomy" somewhere in the text. The examinations are still listed separately under "DETAIL OF ACCREDITATION".

New and old schedules. Schedules are reissued when scope changes. Of the 403, 318 carried a 2026 issue date, 82 a 2025 date and 3 a 2024 date. An older issue date does not mean expired: UKAS says the presence of the schedule is what confirms current accreditation.

5. The numbers from this reading

All figures are from the UKAS directory and schedules read on 20 October 2026, filter Organisation type "Medical Laboratories", searched region by region.

The split by name is only a split by name. Part A includes universities, government agencies, charities, private hospital groups, independent laboratories and partnerships that run laboratory services with NHS trusts. Part B includes NHS trusts and NHS-named services.

6. ISO 15189 entries on the UKAS sanctions page, England

From "Organisations under sanction", updated 19 October 2026, read 20 October 2026. Entries with a town in England only; entries for Abergavenny, Swansea, Glasgow, Dumfries, Melrose, Londonderry, Belfast and Dundonald are left out.

7. The list, part A: organisations whose names do not include "NHS"

121 schedules. Alphabetical by organisation name as UKAS lists it, then by number. "Place and postcode" is the last line of the address in the UKAS directory; a group's other sites are on its schedule. Each schedule link opens the PDF on the UKAS site.

8. The list, part B: organisations whose names include "NHS"

282 schedules, in the same order and with the same columns.

9. How this list was read, and its limits

• Date. Directory searches, all 487 schedules and the sanctions page were read on 20 October 2026. Any of them can change the next day; the schedule on the UKAS site is the record that counts.
• Scope of the search. The UKAS directory filter "Medical Laboratories", run once for each of the 13 regions it offers. A medical laboratory listed under another organisation type would not appear. Every schedule found stated ISO 15189:2022.
• England. Decided from the postcode in the directory's address, checked by hand where the postcode was missing or mistyped. One schedule with a Belfast address whose postcode reads "B12 6BA" was placed in Northern Ireland. A group whose main address is outside England but which has English sites (Randox, Antrim) is not in this list; a group whose main address is in England is listed once, at that address.
• What was copied. Organisation name, accreditation number, address line, issue date and link. Contact names, telephone numbers and email addresses on the schedules were left out.
• What the list does not say. It does not say which laboratory to use, how quickly any laboratory reports, what any laboratory charges, or whether any laboratory accepts samples from a particular clinic. None of that is on a schedule.

What ER Blood does with this
